Continuing Care

NHS Continuing Healthcare is a package of care which is arranged and funded by the NHS for individuals outside hospital who have ongoing healthcare needs. You can receive continuing healthcare in any setting, including your own home or a care home.

National guidance was issued in 2007 (updated 2018) which sets out a single national framework for determining eligibility for NHS continuing healthcare and NHS-funded nursing care. You can view the National Framework here.

There is also a leaflet available which gives a short guide to the process of assessment for eligibility for NHS continuing healthcare and NHS funded nursing care and how decisions are made. 

If you are unhappy with either the process or a decision made regarding eligibility for NHS continuing healthcare, you can request a review of the decision by contacting the CCG. You can access the local resolution policy here.
